Refund Policy

We are committed to customer satisfaction. If you receive the wrong product or a product that is not as anticipated, you may return it for a full refund, provided the following conditions are met:

• Answer Forms, Response Forms, and Manuals: These items must be

returned unmarked and undamaged. A full refund will be issued for complete,
undamaged packets. If a partial packet of Answer Forms or Response Forms is
returned, a prorated refund will be issued based on the percentage of the packet returned.

• All Other Products: All other products must be returned in their original

packaging and in new condition.

• Return Shipping: The customer is responsible for all return shipping costs.

• Return Process: Simply repackage the item securely and mail it to the address below. No prior authorization is needed.
